Roles & Responsibilities

• Conduct thorough evaluations to assess patient communication and swallowing needs
• Provide daily treatments that help patients progress toward meaningful goals
• Manage timely discharges with clear documentation and patient education
• Collaborate with the multidisciplinary therapy team to coordinate care plans
• Meet productivity expectations and follow CRG policies and procedures
• Maintain professional presence on-site to support continuity of care

Qualifications

• Primary skill: SLP (Speech-Language Pathologist)
• 1-3 years of experience (job indicates 1-3 years preferred)
• Prior SNF experience strongly preferred
• Ability to follow facility policies and maintain required productivity
• Willingness to wear black scrubs as dress code

Knowledge & Skills

• Skilled in evaluations, daily treatment planning, and discharge processes
• Experience working within a skilled nursing facility workflow
• Strong documentation skills and familiarity with productivity metrics
• Team-oriented communication and collaboration with nursing and rehab staff
• Ability to follow CRG policies and professional practice standards
