Roles & Responsibilities
- Evaluate and treat residents with speech, language, cognitive-communication, and swallowing disorders to improve function and quality of life
- Develop individualized treatment plans and measurable goals tailored to each resident
- Conduct comprehensive swallowing assessments and implement safe, evidence-based dysphagia interventions
- Document skilled services clearly and timely to meet facility and regulatory requirements
- Collaborate closely with nursing, OT/PT, social work, and families to support coordinated care
- Participate in team meetings and contribute to discharge planning and continuity of care
- Maintain productivity expectations and follow Continuum Rehabilitation Group policies and procedures
- Represent the facility professionally (black scrubs dress code) and be present on-site to receive guaranteed hours
Qualifications
- 1-3 years of clinical experience in a skilled nursing/rehab setting (posting indicates 1-3 years)
- State licensure in New Mexico as a Speech-Language Pathologist (or ability to obtain licensure)
- ASHA Certificate of Clinical Competence (CCC-SLP) preferred
- Experience with dysphagia management and SNF-level documentation requirements
- Strong interpersonal skills and ability to work well within an interdisciplinary team
- Ability to follow employer policies and meet productivity expectations
Education
- Master’s degree in Speech-Language Pathology (typical requirement for SLP roles)
- ASHA CCC-SLP preferred
Knowledge & Skills
- Skilled in assessment and treatment of speech, language, cognition, and swallowing disorders
- Familiarity with SNF regulatory documentation and clinical charting best practices
- Competence with treatment modalities commonly used in post-acute and long-term care
- Strong clinical reasoning, time management, and organizational skills
- Clear, compassionate communication with residents, families, and staff
- Comfortable working in a fast-paced rehab environment and adapting to resident needs
