Roles & Responsibilities
- Conduct thorough speech-language evaluations to build individualized care plans
- Provide daily treatments that help residents improve communication and swallowing safety
- Plan and execute meaningful discharge planning to ensure continuity of care
- Document progress and maintain productivity expectations consistent with facility policy
- Collaborate with the therapy team and nursing staff to coordinate patient-centered care
- Follow CRG policies & procedures and uphold facility standards (dress code: black scrubs)
Qualifications
- 1-3 years of clinical experience (posting indicates 1-3 years; at least 1 year preferred)
- Prior SNF experience strongly preferred
- Active SLP clinical credentials as required by state and facility
- Comfortable meeting productivity expectations (posting notes a 90% productivity requirement)
- Ability to work on-site and follow facility dress code and CRG policies
- Accepts contract travel assignments and can commit to the assignment duration
Knowledge & Skills
- Skilled in speech-language evaluations, daily treatment planning, and documentation
- Experience with dysphagia assessment and safe swallowing strategies in SNF populations
- Strong communication and interdisciplinary collaboration skills
- Familiarity with SNF workflows, discharge planning, and productivity tracking
- Detail-oriented documentation and ability to meet facility productivity standards
