Roles & Responsibilities
- Provide compassionate, high-quality nursing care to residents of the rehab center
- Administer medications and treatments accurately and document per facility policy
- Assess and monitor resident conditions; report changes promptly to the care team
- Support residents with activities of daily living and promote independence
- Collaborate with nurses, therapists and support staff to deliver coordinated care
- Maintain accurate, timely charting in the resident record and uphold confidentiality
- Adhere to infection control, safety protocols and facility standards at all times
Qualifications
- Current Licensed Practical Nurse (LPN) license — must be valid in New Hampshire or obtainable prior to start
- Previous experience in long-term care or rehabilitation settings preferred
- Basic Life Support (BLS/CPR) certification preferred or willing to obtain
- Strong communication skills and ability to work as part of an interdisciplinary team
- Reliable, professional, and compassionate approach to resident care
Education
- Graduate of an accredited practical/vocational nursing program (LPN)
Knowledge & Skills
- Medication administration and documentation best practices
- Vital signs assessment and basic clinical monitoring
- Wound care basics and assistance with treatments
- Experience with electronic medical records (EMR) or charting systems
- Time management, prioritization, and strong clinical judgement
- Clear communication with residents, families and clinical teams
