Roles & Responsibilities

• Provide compassionate direct patient care and assist with activities of daily living
• Assist with bathing, dressing, feeding, and toileting while preserving dignity
• Monitor and record vital signs and report changes to the nursing team
• Support safe patient mobility and transfers with proper body mechanics
• Document care accurately and timely in the facility charting system
• Maintain a clean, safe, and respectful environment for patients
• Communicate effectively with team members and participate in care planning

Qualifications

• Active Licensed Nursing Assistant (LNA) certification (NH required or ability to obtain)
• Current CPR/BLS certification (or willingness to obtain promptly)
• Completion of a state-approved LNA training program
• Ability to pass background check, drug screening, and required health screenings
• Comfort with direct patient care and working as part of a multidisciplinary team
• Dependable, compassionate, and able to follow care plans and directions

Education

• Completion of a state-approved LNA program or equivalent
• High school diploma or GED preferred

Knowledge & Skills

• Strong fundamentals in patient hygiene, feeding, and toileting assistance
• Ability to take and record vital signs and observe for clinical changes
• Knowledge of infection control practices and patient safety principles
• Comfortable with basic charting and electronic documentation
• Good communication and teamwork skills
• Time management and prioritization in a busy care environment
