Roles & Responsibilities

Provide direct patient care including medication administration, wound care, and vital sign monitoring
Collaborate with the healthcare team to develop and implement patient care plans
Monitor patient conditions and report changes to the supervising RN or physician
Maintain accurate and thorough patient documentation
Educate patients and families on health management and post-care instructions
Ensure a safe and clean environment following infection control protocols

Qualifications

Current and active Licensed Practical Nurse (LPN) license in New Mexico
Minimum of 1 year of recent LPN experience preferred
BLS certification from the American Heart Association
Ability to pass a background check and drug screening
COVID-19 vaccination or approved exemption required
Comfortable working 12-hour night shifts in a long-term care setting

Knowledge & Skills

Strong clinical assessment and critical thinking skills
Excellent communication and interpersonal abilities
Proficient in electronic health records (EHR) systems
Knowledge of infection control and safety standards
Ability to work independently and as part of a team
Compassionate approach to patient care
