Roles & Responsibilities

• Perform thorough patient assessments and document findings to support individualized care plans
• Administer medications accurately and timely, following facility protocols
• Provide ADL assistance to promote resident comfort and dignity
• Manage gastrostomy tube care, urinary catheter care, and nebulized treatments as required
• Deliver wound VAC care and CPAP support for residents who need respiratory assistance
• Admit and discharge residents, ensuring smooth transitions and accurate recordkeeping
• Notify MD/NP of pertinent clinical changes and collaborate on care decisions
• Carry out other assigned duties to maintain a safe, therapeutic LTC environment

Qualifications

• Current, active Licensed Practical Nurse (LPN) credential
• Experience working in a Long Term Care (LTC) setting is required
• Demonstrated competency in medication administration and patient assessments
• Experience with ADL assistance, gastrostomy tube care, urinary catheter care, nebulized treatments, wound VAC, and CPAP care
• Ability to admit/discharge residents and communicate effectively with MDs/NPs and interdisciplinary teams
• Proof of flu vaccination or facility-approved medical/religious exception prior to contract start

Education

• Licensed Practical Nurse (LPN) — active state license required

• ADL assistance techniques and resident-centered care
• Gastrostomy tube management and urinary catheter care
• Nebulized treatment delivery and respiratory support (including CPAP)
