Roles & Responsibilities

• Provide compassionate, patient-centered nursing care across assigned caseloads
• Administer medications and treatments accurately and document outcomes
• Perform routine assessments and monitor vital signs, reporting changes promptly
• Collaborate with interdisciplinary team members to coordinate safe patient care
• Maintain accurate, timely documentation in the patient chart/EMR
• Support infection control, safety protocols, and facility policies at all times

Qualifications

• Active LPN (Licensed Practical Nurse) license — Vermont license or ability to obtain prior to start
• Current CPR/BLS certification (or willingness to obtain prior to start)
• Previous LPN experience preferred; comfortable in fast-paced care settings
• Must have own reliable transportation (explicit requirement)
• Ability to pass facility background checks and compliance onboarding

Education

• Graduate of an accredited LPN / practical nursing program
• High school diploma or equivalent

Knowledge & Skills

• Strong medication administration and documentation skills
• Good clinical assessment and basic wound care knowledge
• Familiarity with electronic medical records (EMR) systems
• Time management, organization, and effective communication skills
• Team-oriented mindset and ability to follow care plans and protocols
