Roles & Responsibilities

• Provide patient-centered evaluations and develop individualized treatment plans to restore function and independence
• Deliver skilled therapeutic interventions including therapeutic exercise, gait and balance training, and manual therapy
• Document patient progress clearly and timely in the medical record to support continuity of care
• Collaborate with nurses, physicians, and therapy colleagues to coordinate effective interdisciplinary care
• Educate patients and families on home exercise programs, safety strategies, and recovery expectations
• Participate in discharge planning and communicate functional status and needs for safe transitions
• Adhere to facility policies, safety protocols, and quality improvement initiatives to ensure excellent care

Qualifications

• Licensed Physical Therapist in Pennsylvania or eligible for state licensure
• Graduation from an accredited physical therapy program (DPT preferred)
• BLS/CPR certification (or willingness to obtain prior to start)
• Strong clinical skills in musculoskeletal and functional mobility interventions
• Comfort working in an inpatient rehab/long-term care setting; travel/contract experience a plus

Education

• Doctor of Physical Therapy (DPT) preferred; Bachelor's or Master's in Physical Therapy accepted

Knowledge & Skills

• Skilled in patient assessment, therapeutic exercise, and manual techniques
• Experience with mobility, transfers, and gait training for varied patient populations
• Proficient documentation and use of electronic medical records
• Excellent communication and patient education skills
• Ability to collaborate effectively within an interdisciplinary care team
