Roles & Responsibilities

• Provide compassionate direct patient care and support daily living needs
• Administer medications and treatments accurately and on schedule
• Perform timely assessments and communicate changes in condition
• Document care clearly and thoroughly to support the care team
• Collaborate with nurses, therapists, and families to coordinate care plans
• Maintain a safe, clean environment and follow infection-control practices
• Support residents' emotional well-being and dignity through person-centered care

Qualifications

• Active Licensed Practical Nurse (LPN) or Registered Nurse (RN) credential as listed
• Eligibility or licensure to practice in New Hampshire (as applicable)
• Comfortable working the 3:00 PM - 11:30 PM evening shift
• Experience in long-term care or skilled nursing settings is a plus

• Excellent communication and teamwork abilities
• Reliable documentation and charting practices
• Ability to prioritize tasks in a fast-paced care setting
