Roles & Responsibilities

• Provide compassionate, patient-centered care to residents at every step
• Administer medications and follow physician orders accurately and safely
• Perform routine assessments and monitor vital signs to catch changes early
• Document care clearly and promptly in the patient record to support continuity
• Collaborate with RN team members and other disciplines to deliver coordinated care
• Assist with wound care, ADLs and mobility — helping patients maintain dignity
• Communicate effectively with patients and families, educating and reassuring when needed

Qualifications

• Current Licensed Practical Nurse (LPN) license valid in North Carolina
• Completion of an accredited LPN/LVN program
• Basic Life Support (BLS) card required; additional certifications welcomed
• Clinical experience in long-term care, skilled nursing, or similar settings preferred
• Up-to-date immunizations and ability to pass background/health screenings
• Strong communication skills and a teamwork-first attitude

Education

• Graduate of an accredited LPN/LVN program
• High school diploma or GED

Knowledge & Skills

• Solid knowledge of medication administration and safe documentation practices
• Comfortable performing patient assessments and monitoring vital signs
• Skilled in wound care, ADLs assistance, and basic nursing procedures
• Familiarity with electronic health record (EHR) charting
• Good clinical judgment, time management, and the ability to prioritize tasks
• Strong interpersonal skills for patient/family education and teamwork
