Candidate Pay Rate: $65.8 - $49 per hour

Roles & Responsibilities

Provide home health physical therapy services to patients in the greater Sacramento area
Treat a variety of diagnoses including cardio, pulmonary, orthopedic, neurological, and geriatric conditions
Manage a caseload of 5-6 patients per day
Develop and implement individualized treatment plans
Document patient progress and communicate with the care team

Qualifications

Active Physical Therapist Assistant license in California
Home health experience preferred; new grads considered only in extreme cases
Valid driver's license and reliable transportation
Two supervisory references required (one within 12 months, one within 3 years)
Ability to work independently and manage a schedule

Knowledge & Skills

Knowledge of home health setting and regulations
Skills in treating diverse diagnoses across cardio, pulmonary, orthopedic, neurological, and geriatric populations
Strong communication and organizational skills
Comfort with electronic documentation and patient scheduling
