Roles & Responsibilities

• Provide physical therapy services to patients in their homes, helping them regain independence
• Develop and implement individualized treatment plans under the supervision of a Physical Therapist
• Document patient progress using Oasis and other required systems
• Collaborate with the care team to ensure comprehensive, patient-centered care
• Educate patients and families on exercises and home safety

Qualifications

• Valid Physical Therapy Assistant license in California
• BLS certification required
• Minimum one year of experience as a PTA
• Experience with Oasis documentation required
• Home health experience preferred, but not required

Knowledge & Skills

• Knowledge of Oasis documentation and home health regulations
• Ability to work independently in a home setting
• Strong communication and time management skills
• Compassionate and patient-centered approach
