Roles & Responsibilities
- Provide compassionate end-of-life care to patients and families
- Handle calls appropriately via phone or home/facility visits
- Offer education and emotional support to patients and their loved ones
- Document all calls and visits electronically for continuity of care
- Communicate activity at the end of each on-call shift
Qualifications
- Active RN license in Virginia
- 1+ year of hospice experience
- Valid driver's license with good driving record and reliable transportation
Knowledge & Skills
- Expertise in hospice and palliative care
- Strong patient education and emotional support skills
- Ability to document accurately in electronic systems
- Comfortable with phone triage and on-call responsibilities
