Roles & Responsibilities

• Assess patients' speech, language, cognitive-communication, and swallowing needs using evidence-based evaluation tools
• Develop individualized treatment plans that are meaningful and goal-directed
• Implement therapy interventions and adapt techniques to each patient’s progress and preferences
• Collaborate closely with nursing, PT/OT, physicians, and family members to coordinate care
• Document assessments, treatment plans, and progress notes accurately and timely in the patient record
• Educate patients, families, and caregivers on strategies to support carryover and safety
• Contribute to discharge planning and ensure smooth transitions across care settings
• Participate in team meetings and contribute insights to enhance patient outcomes

Qualifications

• Active or compact-eligible SLP license for California, or ability to obtain licensure quickly
• Master’s degree in Speech-Language Pathology or equivalent graduate degree
• ASHA CCC-SLP preferred but not required
• Experience in post-acute, inpatient rehabilitation, or skilled nursing settings preferred
• Current BLS/CPR certification preferred
• Strong communication and documentation skills with experience using EMR systems

Education

• Master’s degree in Speech-Language Pathology or Communication Sciences and Disorders

Knowledge & Skills

• Skilled in speech, language, cognitive-communication, and dysphagia assessment and treatment
• Knowledge of evidence-based intervention strategies across adult populations
• Proficient documentation and clinical reasoning to drive meaningful outcomes
• Ability to work collaboratively in interdisciplinary teams and mentor caregivers
• Strong patient- and family-centered communication skills
