Roles & Responsibilities

• Provide compassionate, patient-centered physical therapy treatments that help patients regain independence
• Assist with implementing individualized treatment plans under PT direction and document progress clearly
• Collaborate with the interdisciplinary rehab team to create coordinated, effective care
• Perform therapeutic exercises, gait training, transfers, and mobility interventions to meet functional goals
• Communicate progress and educate patients and families to support carryover and independence
• Contribute to a positive, high-energy rehab culture and maintain therapy equipment and a safe treatment environment

Qualifications

• Licensed Physical Therapist Assistant (PTA) — required
• Comfortable working in a skilled nursing facility setting (SNF experience preferred)
• Team player with a positive attitude and passion for helping patients regain independence
• Ability to work the offered schedules: Sunday - Thursday or Tuesday - Saturday

Knowledge & Skills

• Physical therapy principles and hands-on rehabilitation techniques
• Patient rehabilitation and patient-centered care focus
• Experience collaborating within an interdisciplinary healthcare team
• Skilled nursing facility rehabilitation workflows and documentation
