Roles & Responsibilities

Deliver personalized physical therapy services in patients' homes across Yolo County
Assess and evaluate each patient's therapeutic needs, mobility, and home environment
Design and implement customized treatment plans that help patients regain independence
Educate patients and families on therapeutic exercises and safe use of equipment
Collaborate with physicians and care teams to adjust treatment based on patient progress
Document clinical findings and celebrate patient milestones in detailed progress reports

Qualifications

Minimum 2 years of physical therapy experience in acute, home care, or rehab settings
Current BLS certification required
Strong communication and organizational skills
Valid driver's license and reliable transportation for home visits

Knowledge & Skills

Physical Therapy assessment and treatment planning
Home environment evaluation and safety assessment
Therapeutic exercise instruction and gait training
Range of motion and muscle strength evaluation
Patient and family education techniques
Medicare and Medi-Cal compliance knowledge
