Location: Breckenridge, MN

Start Date: 07-06-2026

End Date: 10-05-2026

Contract Type: Travel Contract

Shift Type: Rotating shifts: Days (6:00 AM - 2:30 PM), Evenings (2:00 PM - 11:00 PM), Nights (10:00 PM - 6:30 AM); cycle scheduling, every other weekend

Hours per Week: 36

Candidate Pay Rate: $46 per hour

Roles & Responsibilities

Support the healthcare team by maintaining accurate records and facilitating communication
Coordinate care plans and ensure resident safety
Utilize PointClickCare for accurate charting and documentation
Communicate effectively with residents, families, and team members
Assist with daily activities and ensure a comfortable environment

Qualifications

Active LPN license in Minnesota
Current BLS certification
Experience with PointClickCare preferred
Strong communication and teamwork skills

Knowledge & Skills

Knowledge of long-term care regulations
Skilled in electronic medical records (PointClickCare)
Ability to work in a team-oriented environment
Flexibility to work rotating shifts
Excellent organizational and documentation skills
