Contract Type: Travel Contract

Shift Type: Days, Monday-Friday (start and end times may vary based on patient visits)

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Conduct comprehensive evaluations of patients in their homes
Develop and implement individualized treatment plans to improve mobility and function
Provide therapeutic exercises, modalities, and manual therapy
Document patient progress and communicate with the care team
Educate patients and families on home exercise programs

Qualifications

Active Physical Therapist (PT) license in California
BLS certification
Home Health experience (will consider travelers with less than 2 years if they have home health background)
Ability to travel to patient homes within Tehama County

Knowledge & Skills

Experience with HCHB (HomeCare HomeBase) EMR
Knowledge of orthopedic and geriatric conditions
Excellent communication and organizational skills
Ability to work independently
