Shift Type: Monday - Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Evaluate and treat patients in their homes following surgeries, falls, or chronic conditions
Develop individualized care plans to restore function and independence
Collaborate with a multidisciplinary team to ensure coordinated care
Document patient progress using HomeCare/HomeBase EMR system
Education patients and families on home exercise programs

Qualifications

Current North Dakota Physical Therapy license required
BLS certification required
At least 1 year of recent home health experience
Valid driver's license and auto insurance required

Knowledge & Skills

Expertise in physical therapy assessments and treatments
Familiarity with HomeCare/HomeBase EMR system
Strong communication and time management skills
Ability to work independently in a home health setting
