Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM, rotating weekends and holidays

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Perform start of care, recertifications, evaluations, and daily visits for home health patients
Manage a caseload of patients with diagnoses including CHF, Orthopedic conditions, and Pneumonia
Collaborate with clinical manager and educator for field support
Document patient progress and coordinate with interdisciplinary team

Qualifications

Current Physical Therapist license in Kentucky (or compact eligible)
BLS certification required
Minimum 1 year of home health experience preferred
Willingness to travel to patient homes within the Louisville area

Knowledge & Skills

Strong clinical assessment and treatment planning skills
Excellent communication and patient education abilities
Ability to work independently and manage a daily schedule
Familiarity with home health regulations and documentation
