Location: Louisville, KY

End Date: 09-28-2026

Contract Type: Travel Contract

Shift Type: 5 x 8-hour shifts per week, 8:00 AM - 5:00 PM, with rotating weekends and holiday rotation

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Conduct comprehensive home health evaluations and develop personalized care plans
Perform skilled physical therapy interventions including SOC, ROC, recerts, and daily visits
Educate patients and families on therapeutic exercises and fall prevention
Collaborate with clinical managers and interdisciplinary team for optimal patient outcomes
Maintain accurate documentation and OASIS assessments in a timely manner
Travel to patient homes within the Louisville area to deliver compassionate care

Qualifications

Current Physical Therapist license in Kentucky (or compact eligibility)
BLS certification (required)
Home health experience preferred; new grads welcome to apply
Reliable transportation and valid driver's license
Comfortable with electronic documentation using provided tablet
Excellent communication and interpersonal skills

Knowledge & Skills

Knowledge of home health regulations and OASIS documentation
Strong clinical assessment and critical thinking skills
Ability to work independently and manage time effectively
Proficiency in using electronic health records and mobile devices
