Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Provide skilled physical therapy services to patients in their homes
Develop and implement personalized treatment plans for orthopedic and geriatric patients
Conduct start-of-care visits for appropriate patients when nursing is not ordered
Collaborate with the care team to ensure optimal patient outcomes
Document patient progress using HomeCare HomeBase (HCHB) EMR

Qualifications

Active Physical Therapist license in California (or compact eligible)
Home Health experience required (will consider less than 2 years experience)
Current BLS certification
Ability to travel within Tehama County (approx. 40 miles/day)

Knowledge & Skills

Proficient in HomeCare HomeBase (HCHB) EMR
Strong clinical skills in orthopedics, CHF, COPD, and geriatrics
Excellent communication and patient education abilities
Time management and self-direction in a home health setting
