Shift Type: Monday-Friday, 8:00 AM - 4:30 PM

Hours per Week: 40

Candidate Pay Rate: $74 per hour

Roles & Responsibilities

Conduct comprehensive psychosocial assessments and develop care plans for patients
Coordinate care across multidisciplinary teams to ensure seamless transitions
Provide crisis intervention and conduct 5150 evaluations for involuntary psychiatric holds
Advocate for patients and connect them with community resources
Document patient progress and collaborate with physicians, nurses, and other providers
Support patients and families through mental health challenges with compassion and expertise

Qualifications

Current and valid LCSW license in California
Current BLS certification (American Heart Association)
5150 Certification in California (must have or be willing to obtain prior to start)
Prior experience with Dignity Health or CommonSpirit strongly preferred
Minimum 2 supervisory references (1 within 12 months, 1 within 3 years)
Not directly employed by any CommonSpirit facility within the past year

Education

Master's degree in Social Work from an accredited program

Knowledge & Skills

Expertise in care coordination and discharge planning
Knowledge of mental health evaluation and 5150 processes
Strong communication and interpersonal skills
Ability to work collaboratively in a fast-paced healthcare setting
Proficient in electronic health records (EHR) and documentation
