Roles & Responsibilities

• Provide compassionate, high-quality direct patient care including medication administration and monitoring
• Assess patients regularly and communicate changes in condition to the care team promptly
• Document care accurately and timely in the patient record to support continuity and safety
• Collaborate with nurses, providers, and support staff to deliver coordinated care plans
• Support patient education and discharge planning so patients and families feel confident
• Assist with procedures, wound care, and treatments per facility protocols

Qualifications

• Current Licensed Practical Nurse (LPN) license (Maine) — active and in good standing
• Recent clinical experience as an LPN, preferably in long-term care or similar settings
• Basic Life Support (BLS/CPR) certification — current
• Strong communication skills and a team-first attitude
• Ability to pass facility onboarding and background checks

Education

• Graduation from an accredited practical nursing program or equivalent diploma

Knowledge & Skills

• Medication administration, vital signs, and basic clinical assessments
• Wound care and ADL assistance with a focus on patient comfort and safety
• Electronic health record (EHR) documentation and basic computer skills
• Infection control practices and patient safety protocols
• Effective communication with patients, families, and interdisciplinary teams
