Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents in a rehab setting
- Administer medications and treatments safely and according to policy
- Perform timely assessments and document changes in patient condition
- Collaborate with nurses, therapists, and leadership to create and follow care plans
- Educate residents and families with clarity and empathy
- Maintain a clean, safe environment and follow infection control protocols
Qualifications
- Current LPN license in New Mexico (required)
- BLS/CPR certification (required or obtain upon hire)
- Prior experience in long-term care, rehab, or similar setting preferred
- Strong teamwork, communication, and patient-focused mindset
- Ability to pass required background and health screenings
Education
- Completion of an accredited practical nursing (LPN/LVN) program
- Current state licensure as an LPN
Knowledge & Skills
- Patient assessments and care planning for rehab populations
- Medication administration and documentation best practices
- Wound care and basic clinical procedures common to LPN scope
- Electronic health record (EHR) charting and documentation
- Infection control, safety protocols, and regulatory compliance
- Clear communication with residents, families, and interdisciplinary teams
