Roles & Responsibilities

• Provide direct, compassionate nursing care to residents in a long-term care/rehab setting.
• Administer medications and treatments safely and accurately according to facility policies.
• Monitor and document patient conditions, reporting changes to the care team promptly.
• Collaborate with RNs, CNAs, and interdisciplinary staff to create and follow individualized care plans.
• Assist with admissions, discharges, and transfers while ensuring continuity of care.
• Maintain accurate clinical records and comply with regulatory and facility standards.
• Support residents’ activities of daily living and promote dignity, independence, and comfort.

Qualifications

• Current, active LPN license for practice in New Mexico (required).
• Basic Life Support (BLS/CPR) certification or willingness to obtain upon hire.
• Comfortable working night shifts and managing responsibilities independently on shift.
• Strong communication skills and a collaborative attitude — you’ll be part of a close team.
• Previous LPN experience in long-term care, skilled nursing, or rehab is preferred.

Education

• LPN diploma, certificate, or equivalent practical nursing program completion; state licensure required.

Knowledge & Skills

• Clinical assessment and patient monitoring with attention to detail.
• Medication administration and documentation best practices.
• Familiarity with care planning and interdisciplinary coordination.
• Basic charting and record-keeping; experience with electronic health records is a plus.
• Infection control, safety protocols, and regulatory compliance awareness.
