Roles & Responsibilities
- Assess assigned patients and continuously monitor clinical status to ensure safe, effective care
- Administer medications and manage IV therapy according to orders and best practices
- Monitor cardiac rhythms and respond to changes in condition, escalating to the team as needed
- Develop, implement, evaluate, and revise individualized care plans for assigned patients
- Provide tracheostomy and tube-feeding care for patients with advanced airway and nutritional needs
- Assist patient care technicians with bathing and basic care when needed to ensure dignity and comfort
- Communicate clearly with physicians, family members, and the care team about patient status and care plans
- Complete accurate documentation and participate in required competency testing and screenings
Qualifications
- Active nursing license (license verification required)
- BLS and ACLS certifications required
- Ability to pass VHC EKG exam and other credentialing checks
- Comfort and experience caring for patients with IVs, cardiac monitors, tube feedings, and tracheostomies
- Willingness to complete required health screenings and immunizations (TB, COVID-19, MMR, Hep B, Influenza, Varicella, Tdap, drug screen, fit test)
- Strong communication skills and the ability to collaborate effectively with interdisciplinary teams
Knowledge & Skills
- Patient assessment and ongoing monitoring
- Medication administration and safe documentation
- IV therapy management and troubleshooting
- Cardiac monitoring and basic EKG interpretation
- Tracheostomy care and tube feeding management
- Care planning and care coordination
- Teamwork, communication, and assisting PCTs when needed
- Comfort with required testing and credentialing processes
