Roles & Responsibilities
- Evaluate medically complex patients to identify immediate and ongoing care needs
- Develop and implement functional treatment plans in collaboration with the care team
- Deliver daily patient care and therapeutic interventions, typically 15 to 60 minutes per patient depending on needs
- Maintain accurate EMR documentation for evaluations, treatment progress, and daily notes
- Perform swallow and cognitive assessments and provide evidence-based interventions when appropriate
- Prescribe and recommend clinical diets tailored to patients' swallow dysfunction in coordination with the care team
- Participate in hospital and departmental orientation to ensure safe, consistent patient care
Qualifications
- Valid Licensure/Certificate verification as required for LPN practice
- BLS (Basic Life Support) certification
- Comfortable and experienced with EMR use for evaluations and daily documentation
- Experience working with medically complex patients preferred
- Skills or training in swallow assessment and treatment and cognitive assessment recommended
- Completion of required health screenings and immunizations (COVID-19, TB testing, MMR, Hep B, Fit Test, Influenza, Varicella, Tdap, as listed)
Knowledge & Skills
- Proficient with EMR workflows, charting, and clinical documentation
- Skilled in comprehensive patient evaluation and treatment plan development
- Experience providing speech therapy-related interventions between 15 and 60 minutes as clinically indicated
- Competent in swallow assessment and intervention strategies
- Able to perform cognitive assessments and provide appropriate therapeutic approaches
- Knowledgeable about clinical diet prescription for patients with swallow dysfunction
- Strong communication and interdisciplinary collaboration skills
