Roles & Responsibilities

• Develop individualized plans of care for assigned patients and their families
• Implement, evaluate and revise care plans to promote optimal outcomes
• Provide direct nursing care while delegating and supervising support staff
• Ensure all nursing practice follows applicable regulations and facility standards
• Document patient status, interventions and progress thoroughly and timely
• Educate patients and families about care plans, treatments and discharge needs
• Collaborate with interdisciplinary team members to coordinate patient care
• Participate in unit-based quality improvement and safety initiatives

Qualifications

• Current Licensed Practical Nurse (LPN) license in the state of California
• BLS certification required
• ACLS certification required
• Successful completion of VHC EKG Exam (per checklist)
• License and certificate verification as part of onboarding
• Ability to pass VHC drug screen, TB testing, fit test and required immunizations including COVID-19 and influenza
• Strong clinical judgment, delegation and supervisory skills
• Comfort working in a Long-Term Acute Care (LTAC) environment

Education

• Graduate of an accredited LPN/LVN program and current state LPN license

Knowledge & Skills

• Patient care and bedside nursing skills tailored to complex, long-term patients
• Care planning: assessment, goal-setting and evaluation
• Delegation and supervision of nursing assistants and ancillary staff
• Nurse practice compliance and familiarity with regulatory standards
• Basic EKG interpretation familiarity (VHC EKG Exam required)
• Clear documentation and effective communication with patients, families and the care team
