Roles & Responsibilities
- Develop individualized plans of care for assigned patients and their families
- Implement, evaluate and revise care plans to promote optimal outcomes
- Provide direct nursing care while delegating and supervising support staff
- Ensure all nursing practice follows applicable regulations and facility standards
- Document patient status, interventions and progress thoroughly and timely
- Educate patients and families about care plans, treatments and discharge needs
- Collaborate with interdisciplinary team members to coordinate patient care
- Participate in unit-based quality improvement and safety initiatives
Qualifications
- Current Licensed Practical Nurse (LPN) license in the state of California
- BLS certification required
- ACLS certification required
- Successful completion of VHC EKG Exam (per checklist)
- License and certificate verification as part of onboarding
- Ability to pass VHC drug screen, TB testing, fit test and required immunizations including COVID-19 and influenza
- Strong clinical judgment, delegation and supervisory skills
- Comfort working in a Long-Term Acute Care (LTAC) environment
Education
- Graduate of an accredited LPN/LVN program and current state LPN license
Knowledge & Skills
- Patient care and bedside nursing skills tailored to complex, long-term patients
- Care planning: assessment, goal-setting and evaluation
- Delegation and supervision of nursing assistants and ancillary staff
- Nurse practice compliance and familiarity with regulatory standards
- Basic EKG interpretation familiarity (VHC EKG Exam required)
- Clear documentation and effective communication with patients, families and the care team
