Roles & Responsibilities

• Assess residents’ conditions and create thoughtful care plans in collaboration with the RN
• Deliver direct patient care with compassion, including medication administration and monitoring
• Supervise CNAs and support staff to ensure continuity and quality of care
• Document care accurately and efficiently in the facility’s Electronic Medical Records system
• Communicate clearly and professionally with residents, families, staff, and external agencies
• Participate in care conferences and handoffs to maintain seamless resident care
• Maintain required certifications and pass facility nursing medical testing

Qualifications

• Current California LVN license in good standing
• Current BLS certification
• Required: long-term care / skilled nursing experience
• Experience with Electronic Medical Records (EMR) — it’s a must
• Comfort supervising CNAs and coordinating support staff
• Effective English communication skills, both written and verbal
• Basic computer skills and ability to complete electronic documentation

Education

• Completion of an accredited Vocational Nursing program and active LVN license

Knowledge & Skills

• Patient assessment and care planning
• Medication administration and safe documentation
• Hands-on direct patient care in a skilled nursing setting
• Proficiency with Electronic Medical Records (EMR)
• Ability to supervise and mentor CNAs
• Strong interpersonal skills and professional communication
• Basic computer literacy (charting, email, electronic forms)
