Roles & Responsibilities
- Provide compassionate, patient-centered nursing care to residents in a rehab setting
- Administer medications and treatments accurately and on time
- Assess and monitor patient condition, reporting changes to the care team
- Document care thoroughly and maintain clear, timely charts
- Collaborate with nurses, therapists, and physicians to develop and follow care plans
- Support patients and families with education, encouragement, and empathy
- Assist with admissions, discharges, and transitions of care to ensure smooth handoffs
Qualifications
- Current LPN license in New Mexico (required)
- Completion of an accredited LPN/LVN program
- Basic Life Support (BLS) certification from an accredited provider (required)
- Previous LPN experience in long-term care, rehab, or acute care preferred
- Ability to pass background check and drug screen per facility policy
- Strong communication skills and reliable attendance
Education
- LPN/LVN diploma or certificate from an accredited program
- High school diploma or GED
Knowledge & Skills
- Skilled in medication administration and basic nursing procedures
- Patient assessment and vital sign monitoring
- Wound care and simple dressing changes
- Comfortable with electronic medical records and clear documentation
- Strong teamwork, time management, and prioritization
- Focused on patient safety, infection control, and quality care
