Roles & Responsibilities

• Provide compassionate direct patient care, monitoring vital signs and responding to changes promptly
• Administer medications and treatments accurately and document all care in the chart
• Collaborate with RNs, physicians, and care team members to implement individualized care plans
• Assist with admissions, discharges, and transfers to ensure smooth transitions for residents
• Communicate clearly and empathetically with residents and families, advocating for patient needs
• Maintain a safe, clean environment and follow infection control and facility policies

Qualifications

• Current LPN license (Licensed Practical Nurse) — appropriate state licensure required
• Experience working in long-term care or skilled nursing is a plus; new grads who are confident and caring are welcome to apply
• Ability to work the assigned night schedule (7:00 PM - 7:30 AM) including weekends (Fri, Sat, Sun)
• Strong communication skills and the ability to collaborate with a multidisciplinary team
• Current basic life support (BLS/CPR) certification preferred or willing to obtain

Education

• Graduate of an accredited Practical Nurse program
• High school diploma or GED

Knowledge & Skills

• Solid clinical assessment skills and comfort with medication administration
• Familiarity with charting and documentation best practices
• Ability to prioritize tasks in a fast-paced environment and respond to resident needs
• Compassionate bedside manner and strong interpersonal skills for resident-centered care
• Team-oriented approach and willingness to support colleagues during busy shifts
