Roles & Responsibilities

• Provide compassionate direct patient care to residents in a nursing and rehab setting
• Administer medications and treatments accurately and document appropriately
• Perform timely assessments and communicate changes in condition to the care team
• Support rehabilitation goals through active participation in care plans
• Maintain clear, professional documentation and handoffs at shift change
• Collaborate with nurses, therapists and other staff to deliver coordinated, person-centered care

Qualifications

• Active Licensed Practical Nurse (LPN) credential (LPN) required
• Travel LPN (T-LPN) classification supported for this assignment
• Comfort and experience working 12-hour night shifts
• Current basic life support (BLS) or CPR certification preferred or easily obtainable

Education

• LPN diploma or vocational nursing certificate

Knowledge & Skills

• Strong clinical assessment and medication administration skills
• Familiarity with rehabilitation nursing and long term care best practices
• Excellent documentation and charting habits
• Clear communication and teamwork skills to collaborate with interdisciplinary teams
• Time management and prioritization during busy nightshift periods
