Roles & Responsibilities
- Administer medications and manage IV therapy safely and accurately
- Assess patients, develop and implement individualized plans of care, and evaluate outcomes
- Provide tracheostomy care and manage enteral/tube feedings as needed
- Monitor cardiac status and respond appropriately to changes
- Assist and collaborate with patient care technicians (PCTs) and other staff to ensure high-quality care
- Communicate effectively with physicians and patient families by phone and documentation
- Maintain accurate license and certification verification and comply with required health screenings
Qualifications
- Valid LPN license (must be active and verifiable)
- BLS certification required
- Comfortable and competent with medication administration, IV management, cardiac monitoring, tracheostomy and enteral feeding care
- Ability to perform patient assessments and develop care plans
- Willingness to complete required health screenings and immunizations listed by the facility (COVID-19 vaccine, TB test/questionnaire, drug screen, MMR, Hep B, Influenza, Varicella, Tdap, fit test, color vision, health statement)
- Strong communication skills for phone interaction with physicians and families, and teamwork-oriented attitude
Education
- Graduate of an accredited practical nursing program or equivalent LPN diploma/certificate
Knowledge & Skills
- Medication administration and safe documentation practices
- IV therapy setup, monitoring and troubleshooting
- Cardiac monitoring interpretation basics
- Tracheostomy care and suctioning techniques
- Enteral feeding management and tube care
- Patient assessment, care planning and clinical judgment
- Collaborative teamwork and clear phone communication with clinicians and families
