Roles & Responsibilities

• Provide compassionate bedside care to residents, ensuring dignity and comfort
• Administer medications and treatments accurately and document according to policy
• Monitor and record vital signs, recognizing and escalating changes in condition
• Collaborate with nurses, therapists and physicians to deliver coordinated care
• Support rehabilitation activities and assist with ADLs to promote patient independence
• Maintain clear, timely clinical documentation in the electronic medical record
• Participate in shift handoffs and team care planning to ensure continuity

Qualifications

• Current Licensed Practical Nurse (LPN) licensure (KY or ability to obtain state licensure)
• Active Basic Life Support (BLS) or CPR certification (current) preferred
• Comfortable working 12-hour day shifts with weekend availability
• Strong patient care skills and a team-first attitude
• Previous long-term care or rehab experience is a plus

Education

• Graduate of an accredited LPN/LVN program; relevant certifications as required by state

Knowledge & Skills

• Solid clinical assessment and medication administration skills
• Familiarity with infection control and wound care basics
• Comfortable with documentation and charting in an EMR
• Clear communication and the ability to collaborate with interdisciplinary teams
• Good time management and prioritization during busy shifts
