Roles & Responsibilities

• Evaluate patients’ functional mobility and design individualized rehabilitation plans
• Deliver skilled therapeutic interventions to restore movement and independence
• Collaborate with interdisciplinary teams to coordinate continuity of care
• Document patient progress clearly and timely to support outcomes and transitions
• Educate patients and families on home programs, injury prevention, and recovery goals
• Travel between outpatient care sites as needed to meet patient needs

Qualifications

• Current, unencumbered Oregon Physical Therapy license (required)
• Healthcare Provider BLS certification (required)
• Experience in outpatient physical therapy or related rehab settings preferred
• Comfortable traveling between multiple points of care within the service area
• Strong communication, documentation, and patient-education skills

Knowledge & Skills

• Outpatient Physical Therapy evaluation and therapeutic interventions
• Rehabilitation treatment planning for orthopedic, pediatric, neurological and balance conditions
• Sports and orthopedic rehab techniques and modalities
• Neurological and stroke rehab approaches and adaptive strategies
• Wheelchair seating assessments and mobility device recommendations
• Neck, back and extremity care with functional restoration focus
