Roles & Responsibilities

• Provide compassionate, hands-on patient care and support daily needs
• Administer medications and treatments safely and accurately per orders
• Monitor, assess and document vital signs, patient status and changes
• Collaborate with nurses, therapists and providers to deliver coordinated care
• Educate patients and families about care plans and home needs
• Maintain thorough, timely electronic health record documentation
• Respond swiftly to patient needs and assist in emergency situations
• Adhere to infection control, safety protocols and facility policies

Qualifications

• Active LPN (Licensed Practical Nurse) license or eligible for licensure in the state of practice
• Current BLS/CPR certification (or ability to obtain prior to start)
• Clinical experience providing direct patient care in a long-term care or rehab setting preferred
• Strong communication skills and ability to work well within an interdisciplinary team
• Reliable, professional attitude with attention to detail and patient safety

Education

• Diploma or certificate from an accredited practical nursing program (LPN/Vocational Nursing)

Knowledge & Skills

• Patient assessment and basic nursing care
• Medication administration and documentation
• Comfort with electronic health record systems and documentation best practices
• Wound care fundamentals and basic infection control
• Time management, prioritization and effective communication
• Team collaboration and patient/family education
