Roles & Responsibilities
- Evaluate patients' functional status and develop individualized occupational therapy plans of care
- Complete thorough assessments and reassessments using objective tests and measurements
- Deliver home-based therapy that helps patients regain independence in self-care, leisure, and work activities
- Document care accurately and concisely, capturing treatments, communications, and coordination
- Communicate treatment observations and recommendations as an active member of the interdisciplinary team
- Provide patient and caregiver education to support safe home functioning and ongoing recovery
- Adapt quickly to changes in patient condition and respond to emergent needs with clinical judgment
- Respect and protect patient rights while conveying empathy and support during difficult moments
Qualifications
- Licensed by the State of Oregon as an Occupational Therapist (required)
- Graduate of an accredited occupational therapy curriculum and eligible for the National Registration Examination of the American Occupational Therapy Association
- Current certification in BLS (required)
- Valid driver’s license and proof of auto insurance (required) — must have own car for patient home visits
- Registration with the American Occupational Therapy Association preferred
- CRASE training required if travel assignment extends beyond six months (preferred)
- Evidence of continuing education in physical disability/rehabilitation, medicine and surgery procedures, mental health concepts, geriatric and long-term care (desired)
Education
- Graduate of an occupational therapy curriculum accredited by the Committee on Allied Health Education and Accreditation (required)
- Eligible for the National Registration Examination of the American Occupational Therapy Association (required)
Knowledge & Skills
- Patient assessment and objective measurement for home health and hospice settings
- Treatment planning and implementation focused on meaningful daily activities
- Clinical documentation and accurate communication across the care team
- Care coordination and interdisciplinary team collaboration
- Home visit best practices, safety assessment, and adaptive equipment recommendations
- Patient and caregiver education with a compassionate, person-centered approach
- Experience with geriatric care, long-term care needs, and mental health concepts relevant to OT
