Roles & Responsibilities

• Provide compassionate in-home nursing care to patients on the Calendonia Home Health and Hospice caseload
• Perform comprehensive assessments, create individualized care plans, and adjust interventions as needed
• Administer medications, manage wounds, and perform skilled nursing procedures safely and effectively
• Educate patients and caregivers on disease management, medication administration, and symptom control
• Collaborate with interdisciplinary team members to coordinate care and ensure smooth transitions
• Document patient encounters accurately and promptly in the clinical record
• Advocate for patient needs and work to maintain quality of life in the home setting

Qualifications

• Current, unrestricted RN license (Vermont) or compact RN license eligible — active licensure required
• Experience in home health and/or hospice care preferred; comfort with home visits is essential
• Basic Life Support (BLS) certification required; additional certifications in hospice or palliative care a plus
• Strong assessment and clinical decision-making skills with the ability to work independently
• Valid driver's license, reliable transportation, and willingness to travel within the service area
• Excellent communication and interpersonal skills for working with patients, families, and the care team

Education

• Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN); BSN preferred

Knowledge & Skills

• Clinical assessment and care planning for home-bound patients
• Skilled nursing procedures including medication administration and wound care
• Knowledge of hospice principles and symptom management
• Patient and caregiver education techniques
• Time management and caseload prioritization for efficient home visit scheduling
• Familiarity with electronic medical records and accurate documentation practices
