Roles & Responsibilities

• Perform thorough physical therapy evaluations and create individualized treatment plans
• Deliver therapeutic interventions to improve mobility, strength, and function
• Collaborate with nursing and interdisciplinary staff to coordinate patient care and discharge plans
• Educate patients and families on home programs, injury prevention, and realistic recovery goals
• Document patient assessments, plans, and progress clearly and timely in the medical record
• Contribute to a positive rehabilitation environment and participate in quality improvement efforts

Qualifications

• Current California Physical Therapist license or eligibility to obtain one
• Basic Life Support (BLS) certification required
• Clinical experience in outpatient, inpatient rehab, or skilled nursing preferred
• Ability to work the assigned schedule including weekends as needed
• Clear background check and up-to-date immunizations per facility policy

Education

• Doctor of Physical Therapy (DPT) or Bachelor/Master degree in Physical Therapy from an accredited program

Knowledge & Skills

• Proficient in therapeutic exercise, manual therapy, and functional mobility training
• Strong patient assessment and clinical reasoning skills
• Excellent documentation and time management to handle a full caseload
• Comfortable working with diverse and medically complex patient populations
• Effective communicator and collaborative team member
• Familiarity with discharge planning and community resource coordination
