Roles & Responsibilities

• Coordinate patient care across settings — ensure smooth transitions from inpatient to home or facility
• Conduct comprehensive initial assessments to develop individualized discharge plans
• Arrange services like home health, DME, nursing home placement, and prior authorizations
• Manage a caseload (typically about 15–28 patients) with attention to quality and efficiency
• Document care thoroughly in Cerner to keep the team informed and patients safe
• Liaise with payers and community resources to support uninsured or homeless patients
• Collaborate with interdisciplinary teams to remove barriers to discharge and optimize outcomes

Qualifications

• Active nursing license required (license type to submit was indicated in the posting)
• Minimum 3 years nursing experience in an acute hospital setting
• Prior care coordination or House Supervisor experience in a CommonSpirit / Dignity / CHI hospital (must-have)
• BLS Certification required
• Strong customer service, interpersonal and written communication skills
• Ability to multitask, work independently, and thrive in a fast-paced environment
• Experience with discharge planning, prior authorizations, DME coordination, and home health arrangements

Education

• Nursing degree (ADN or diploma acceptable); BSN not required per posting
• Completion of required orientation hours (posting specifies orientation included)

Knowledge & Skills

• Proficient with Cerner EMR charting and documentation
• Inpatient acute nursing and case management best practices
• Discharge planning workflows and care coordination processes
• Comfort working with diverse payer mixes, including uninsured and homeless populations
• Skilled at organizing competing priorities and managing a high caseload
• Effective communicator with patients, families, payers, and clinical teams
