Roles & Responsibilities

• Provide compassionate, patient-centered nursing care during 12-hour night shifts
• Administer medications, monitor vital signs, and document patient status accurately
• Collaborate with interdisciplinary team members to develop and follow care plans
• Perform assessments and report changes in patient condition in a timely manner
• Support safe transitions of care during shift handoffs and discharge planning
• Maintain accurate electronic health record documentation and complete required charting
• Adhere to facility policies, infection control practices, and regulatory standards

Qualifications

• Active LPN (Licensed Practical Nurse) license in the state of practice or compact equivalency
• Current BLS (Basic Life Support) certification or ability to obtain upon hire
• Current immunizations and TB test per facility policy and successful background check
• Comfortable and experienced with 12-hour night shift responsibilities
• Strong communication skills and a team-first mindset
• Previous experience in long-term care, med-surg, or similar settings is a plus

Education

• Graduation from an accredited Practical Nursing program and current LPN licensure

Knowledge & Skills

• Medication administration and accurate dose documentation
• Basic wound care and dressing changes
• Familiarity with electronic health records and computerized charting
• Ability to perform patient assessments and recognize changes in condition
• Infection control practices and safe patient handling
• Time management and prioritization during busy night shifts
