Roles & Responsibilities

• Provide compassionate bedside care to residents, focusing on safety, dignity, and comfort
• Administer medications and treatments accurately and document all care in the medical record
• Monitor patient status, recognize changes, and escalate concerns to the care team promptly
• Collaborate with nurses, therapists, and interdisciplinary staff to create and follow individualized care plans
• Perform routine clinical tasks such as wound care, vital signs, and ADL assistance with professionalism
• Educate residents and families about care plans and provide reassurance and clear communication
• Maintain infection control standards and follow facility policies to ensure a safe environment

Qualifications

• Active Licensed Practical Nurse (LPN) license (LPN required; travel LPN/T-LPN accepted)
• Eligible to work as a travel nurse and complete required onboarding for the facility
• Basic Life Support (BLS) / CPR certification preferred or obtained upon hire
• Current immunizations and TB screening per facility policy
• Comfortable working independently on night shift and collaborating with the care team

Education

• Diploma or certificate in Practical Nursing (LPN) and active state licensure

Knowledge & Skills

• Strong medication administration and documentation skills
• Good clinical assessment and observation abilities
• Proficiency with electronic medical records and charting
• Excellent communication and teamwork skills
• Ability to prioritize tasks and manage time effectively during busy shifts
• Commitment to resident-centered care and safe nursing practices
