Shift Type: Monday-Friday (5x8-hour), 8:00 AM - 4:30 PM, Day shift (includes every-other-weekend rotation to meet department needs)
Hours per Week: 40
Candidate Pay Rate: $67 per hour

Roles & Responsibilities
- Perform comprehensive care coordination and discharge planning for acute care patients
- Manage an assigned caseload with an indicated ratio of approximately 1:20
- Document thoroughly and accurately using Epic and required computerized charting systems
- Coordinate transitions of care including DME, SNF placement, and ambulance transport independently
- Apply InterQual criteria, CMS/DRG knowledge, and utilization review principles to support appropriate care
- Collaborate with interdisciplinary team members (social services, transportation, clinicians) to develop and execute plan of care
- Facilitate family meetings and participate in treatment planning to ensure patient-centered outcomes
- Support regulatory compliance and participate in departmental quality initiatives
Qualifications
- California state licensure required (pending license accepted but must be licensed upon start date)
- Minimum 2 years of acute care hospital case management experience performing care coordination and discharge planning (hospital experience is a must)
- Travel experience required; first-time travelers accepted and will be screened by clinical
- Proficient with Epic and other charting systems; comfortable with computerized documentation
- Experience coordinating DME, SNF, ambulance transport and working with HMOs/IPAs
- Ability to organize and lead family meetings and collaborate across disciplines
- Willingness to participate in weekend rotation and meet department scheduling needs
Knowledge & Skills
- Strong knowledge of care coordination, discharge planning, and plan of care development
- Familiarity with InterQual criteria and CMS/DRG regulatory frameworks
- Skilled in Epic electronic health record and general charting systems
- Competent with Microsoft Office applications
- Excellent communication, organization, and case management prioritization skills
- Experience navigating social determinants of health: homelessness, substance use, mental health crises, advance directives/POLST, protective services
