Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents and patients
• Administer medications and treatments accurately and safely according to orders
• Monitor, document and report changes in patient condition promptly
• Collaborate with interdisciplinary team members to develop and follow care plans
• Educate patients and families on care needs, medications and discharge instructions
• Maintain accurate nursing documentation and comply with facility policies and regulations
• Demonstrate professional judgement and prioritize patient safety at all times

Qualifications

• Current LPN license in Pennsylvania (if applying as LPN) — RN applicants must hold a valid Pennsylvania RN license
• BLS/CPR certification required (or obtain upon hire as agreed with facility)
• Recent clinical experience in long-term care, skilled nursing, or acute care preferred
• Excellent communication skills and a team-oriented mindset
• Ability to pass required background checks and meet facility onboarding requirements

Education

• Graduate of an accredited LPN program (or accredited RN program if applying as RN)
• High school diploma or GED required; Associate's or Bachelor's degree in Nursing for RN applicants as applicable

Knowledge & Skills

• Solid medication administration and basic clinical assessment skills
• Familiarity with electronic health records and accurate charting
• Knowledge of infection control practices and patient safety standards
• Strong time management and prioritization in a fast-paced environment
• Ability to build rapport with patients, families and care team members
