Roles & Responsibilities

• Provide compassionate, high-quality nursing care to residents in a skilled rehab setting
• Perform thorough patient assessments and develop individualized care plans
• Administer medications and treatments safely and accurately
• Collaborate with interdisciplinary team members to coordinate care and improve outcomes
• Document care and communicate changes in patient status in the chart promptly
• Support patient and family education to promote recovery and independence
• Assist with admissions, discharges, and transitions of care with professionalism

Qualifications

• Current RN license in New Hampshire or eligibility for state licensure (compact license accepted if applicable)
• Basic Life Support (BLS) certification required; ACLS/PALS as facility prefers or desired
• Experience in skilled nursing, rehab, or long-term care settings preferred
• Current immunizations and ability to pass background and health screenings
• Strong clinical judgment, excellent communication, and teamwork skills

Education

• Associate Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) preferred

Knowledge & Skills

• Skilled in patient assessment, medication administration, and wound care
• Comfortable with EMR charting and timely documentation
• Familiarity with rehab nursing practices and interdisciplinary care planning
• Ability to prioritize, multitask, and respond calmly in urgent situations
• Strong patient/family education and discharge planning abilities
