Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in a skilled nursing and rehab setting
• Perform assessments, document status changes, and communicate clearly with the care team
• Administer medications and treatments safely and according to facility policies
• Support resident mobility and daily living activities while promoting dignity and independence
• Collaborate with RNs, therapists, and providers to implement individualized care plans
• Educate residents and families about care plans and respond promptly to concerns

Qualifications

• Current Massachusetts Licensed Practical Nurse (LPN) license required (Registered Nurse applicants welcome if applying under RN classification)
• Current BLS/CPR certification required or must obtain upon hire
• Experience in long-term care or skilled nursing is a plus and helpful but not strictly required
• Comfortable with medication administration, documentation, and basic wound care
• Ability to pass background check and meet facility onboarding requirements
• Strong communication skills and a team-first mindset

Education

• Graduate of an accredited LPN program; high school diploma or equivalent required
• Completion of state-approved nursing coursework and clinical training

Knowledge & Skills

• Knowledge of long-term care and rehabilitation nursing practices
• Ability to perform clinical assessments and safely implement nursing interventions
• Familiarity with medication administration and documentation standards
• Good interpersonal skills to connect with residents, families and interdisciplinary team members
• Time management and prioritization in a busy care environment
