Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents in a skilled nursing and rehab setting
- Perform assessments, document status changes, and communicate clearly with the care team
- Administer medications and treatments safely and according to facility policies
- Support resident mobility and daily living activities while promoting dignity and independence
- Collaborate with RNs, therapists, and providers to implement individualized care plans
- Educate residents and families about care plans and respond promptly to concerns
Qualifications
- Current Massachusetts Licensed Practical Nurse (LPN) license required (Registered Nurse applicants welcome if applying under RN classification)
- Current BLS/CPR certification required or must obtain upon hire
- Experience in long-term care or skilled nursing is a plus and helpful but not strictly required
- Comfortable with medication administration, documentation, and basic wound care
- Ability to pass background check and meet facility onboarding requirements
- Strong communication skills and a team-first mindset
Education
- Graduate of an accredited LPN program; high school diploma or equivalent required
- Completion of state-approved nursing coursework and clinical training
Knowledge & Skills
- Knowledge of long-term care and rehabilitation nursing practices
- Ability to perform clinical assessments and safely implement nursing interventions
- Familiarity with medication administration and documentation standards
- Good interpersonal skills to connect with residents, families and interdisciplinary team members
- Time management and prioritization in a busy care environment
