Roles & Responsibilities
- Provide thoughtful, hands-on nursing care to residents in a skilled rehab and long-term care setting
- Administer medications and treatments accurately and document thoroughly
- Assess resident condition, report changes promptly, and collaborate with the interdisciplinary team
- Support rehabilitation plans and assist residents with activities of daily living to promote independence
- Educate residents and families with compassion and clear communication
- Maintain accurate clinical documentation and follow facility policies to ensure high-quality care
Qualifications
- Active LPN or RN license (you’ll need the appropriate state license to practice)
- Current CPR/BLS certification
- Prior experience in long-term care or rehab settings preferred but caring attitude and willingness to learn are key
- Strong communication skills and the ability to work well in a team
- Current immunizations and any facility-required health screenings
Education
- Completion of an accredited practical nursing program for LPNs or an accredited nursing program for RNs
Knowledge & Skills
- Solid clinical assessment and vital-signs monitoring skills
- Medication administration and documentation best practices
- Comfortable supporting rehab-focused care plans and restorative nursing
- Good charting skills and familiarity with electronic health records (or willingness to learn)
- Empathy, patience, and excellent interpersonal skills for resident-centered care
