Roles & Responsibilities
- Provide compassionate, high-quality nursing care to residents in a rehab/long-term care setting
- Administer medications and treatments safely and in accordance with facility policies
- Perform thorough patient assessments and monitor vital signs throughout your shift
- Document care accurately and clearly to ensure continuity across the care team
- Collaborate with nurses, therapists, and physicians to support resident progress and discharge planning
- Educate residents and families about care plans, medications, and recovery goals
- Support a positive, resident-centered environment that promotes dignity and independence
Qualifications
- Active Licensed Practical Nurse (LPN) license — travelers (T-LPN) are welcome
- Current BLS/CPR certification (or willingness to obtain prior to start)
- Prior experience in long-term care or rehabilitation preferred
- Strong clinical judgment, attention to detail, and excellent communication skills
- Ability to work scheduled 12+ hour day shifts (6:00 AM - 6:30 PM) on assigned days
Education
- Graduate of an accredited Practical Nursing program (LPN/LVN diploma or certificate)
Knowledge & Skills
- Proficient in medication administration and documentation
- Skilled in basic wound care, ADL assistance, and resident assessments
- Comfortable using electronic medical records and clear charting practices
- Strong time management and prioritization during busy shifts
- Collaborative team player with a resident-first mindset
