Roles & Responsibilities

• Provide compassionate, high-quality nursing care to residents in a rehab/long-term care setting
• Administer medications and treatments safely and in accordance with facility policies
• Perform thorough patient assessments and monitor vital signs throughout your shift
• Document care accurately and clearly to ensure continuity across the care team
• Collaborate with nurses, therapists, and physicians to support resident progress and discharge planning
• Educate residents and families about care plans, medications, and recovery goals
• Support a positive, resident-centered environment that promotes dignity and independence

Qualifications

• Active Licensed Practical Nurse (LPN) license — travelers (T-LPN) are welcome
• Current BLS/CPR certification (or willingness to obtain prior to start)
• Prior experience in long-term care or rehabilitation preferred
• Strong clinical judgment, attention to detail, and excellent communication skills
• Ability to work scheduled 12+ hour day shifts (6:00 AM - 6:30 PM) on assigned days

Education

• Graduate of an accredited Practical Nursing program (LPN/LVN diploma or certificate)

Knowledge & Skills

• Proficient in medication administration and documentation
• Skilled in basic wound care, ADL assistance, and resident assessments
• Comfortable using electronic medical records and clear charting practices
• Strong time management and prioritization during busy shifts
• Collaborative team player with a resident-first mindset
